Patient Portal

Click the Patient Portal login link below to access you account
  • SGF Patient Portal

Your user name for the patient portal is your email address. If you are experiencing an issue outside of those outlined above, call your local office or nurse/clinical team for more assistance.

  • Step 1: To reset your password, click the “forgot password” link found below the username and password fields when logging into the portal.
  • Step 2: You will receive an email with a link to reset your password. If you did not receive this email, check your spam, junk, and All Mail (gmail) folders. We have found the email often is there.
  • Step 3: Once you have found the email, follow the prompts to reset your password.
  • Step 4: If you did not receive an email, reach out to your SGF office and speak with the front desk to further address the issue.

Forms

Before your appointment, you will receive an email and text message from Phreesia, our private, HIPAA-compliant, electronic patient intake system, prompting you to complete your New Patient paperwork. If your New Patient paperwork is not submitted three business days before your appointment, it will be canceled and need to be rescheduled.

NOTE:  There is a charge of $6.50 flat fee for all records delivered electronically. For records delivered via mail there is a fee per page + $0.90 processing fee + tax + postage for records delivered via mail. CIOX Health has been contracted to provide this service and will invoice you directly.  Please do not send payment to SGF.  Please allow up to 30 days for records to be processed.
