Your first visit
The initial fertility consult with your physician is a comprehensive evaluation of your history and medical records. The appointment typically lasts 45 to 90 minutes. During this visit, you will have the opportunity to have your questions answered and to participate in developing a plan for further evaluation and/or treatment. We strongly that encourage both partners attend the fertility consult.
What to expect during your fertility consult
To make the most of your first visit, please plan to arrive a minimum of 15 minutes prior to your scheduled appointment time in order to complete any necessary paperwork. Be sure to look at your local office webpage to prepare for any special parking considerations.
Upon arrival you will check in with a patient service representative (PSR). During this time, the PSR will collect your insurance information, ID, and take your picture for your electronic medical record.
After checking in, you will meet with two key members of your health care team: your physician and nurse. As part of your initial consultation, patients will also speak with their financial counselor. The purpose of this meeting is to review the benefits our financial department obtained from your insurance provider prior to your appointment. During this time, we will also review any financial questions you may have. In most cases, this meeting is a phone consultation that you are able to schedule online. You will receive an email with the link to schedule an appointment within 2 business days of your new patient visit.
In most cases, you will not complete any testing or have a physical exam during your initial consult. In other cases, if you’re on or near day 3 of your menstrual cycle and prior authorization requirements have been met, your physician may give you the option of initiating testing during this appointment or wait until the following month.
Fertility consult checklist
Preparedness will help to ensure that your visit runs smoothly. Therefore, please prepare the following checklist items before you arrive for your fertility consult. You will need to bring the following with you to your appointment:
- Completed male and female medical questionnaires, registration form, and genetic counseling paperwork
- A copy of your medical records from your OB/GYN, primary care physician, or another fertility program
- Medical records from the urologist, if applicable
- Hysterosalpingogram (HSG) report and film from the hospital or radiology laboratory, if applicable
- Insurance cards for both partners
- A referral from your primary care physician, if applicable
- Insurance authorizations, if applicable
We also encourage you to write down your questions in advance of your appointment and bring your questions with you. During the initial consult, there is a lot to discuss; this way you’re more likely to make the most of the time spent with your physician.
